Withdrawing from a class can have a significant impact if you are a student athlete, international student, or receiving VA education benefits. It can also affect on campus housing, financial aid, and scholarships.  Please make sure to speak with your instructor before dropping a course.  

By withdrawing from a class, timeliness to degree completion may also be affected.  
I acknowledge that it is my responsibility to discuss withdrawing from my course(s) with my relevant departments.
Courses to Drop
Please indicate the following so we may initiate a withdraw on your behalf.

Once you have completed this page, this form will be routed to your advisor and the drop process initiated. You will be notified by the Office of the Registrar once the process has been completed, it may take 2-3 days before you are notified.  Your advisor may contact you with questions about your withdrawal as well. If this form is submitted after 5 pm, on a Friday, during a holiday or a weekend, the process may not be started until the next business day.  
ex. ANSC 100. If you are dropping a course with a lab or other component, please make sure to include it as well.
We would like to know the reason behind your drop. This information may help provide resources and assistance to students as well as feedback to the departments and/or instructors offering the course. No names will be attached to any comments.

All withdrawals are processed by the Registrar’s Office and will require confirmation from your instructor. If this form is submitted after 5 pm, during a holiday or a weekend, the process will not be started until the next business day.
